Characteristics of the Care Environments in Which Advanced Practice Registered Nurses (APRNs) Practice and How These May Impact Revenue

            Advanced practice registered nurses such as nurse practitioners practice in diverse settings. One of these settings is a community health center where the nurse practitioner basically offers primary health care services to the surrounding community. These are basic healthcare services for common diseases that do not require complicated management. This paper examines two conditions in a community health center that may affect revenue and how the APRN provides care in the setting.Resource Management in Primary Care

The first condition is that majority of the community members that receive healthcare services at the health center are covered by the Centers for Medicare & Medicaid Services (CMS). They are patients who are beneficiaries of the Patient Protection and Affordable Care Act 2010. As such, the amount of reimbursement for any care received by these community members from this health center is based on the quality of the service received as per the pay-for-performance policy. The problem is that as a community health center, the facility may not have all the equipment or resources that would enable the NP to provide the best of care. As a result, the revenues will not be as high as would be expected. The ability of the APRN to give only the best care is thus compromised (Joel, 2009).

The second condition in this care setting is that it is a point of first access or primary health care facility. This means that the range of services that can be provided in the community health center is limited. Because of this, the amount of revenue that can be generated by the limited services offered at this health center will also be limited. The APRN is impacted in how she can provide care in this setting in that she has to refer upwards to better equipped facilities any cases that she may not handle because of finite resources.
